How to register

If you have just moved to the area and now live in Post Code WA3 1,2,or 3; or you don’t currently have a doctor in the area then we welcome your registration.  This can be done in person and our friendly reception staff will provide you with the paperwork and make you an appointment with our Practice Nurse for a New Patient Medical.

Due to Covid-19 and Social Isolation/Lock down we do not want any patients coming into the building and with this in mind if you wish to register you can use the registration links below for  – GMS1 (this is mandatory), New Patient Form, and online services. We are suspending new patient medicals in person for the foreseeable future, however the nurse or HCA may contact you by telephone.

  3. Register online to order your repeat prescriptions, book GP appointments, and access your medical record. You can do this by telephone or contact us by email and the practice will send the link directly to your email address.

If you move out of the area and wish to stay registered please be aware that we have an outer boundary. The boudary takes in part of Wigan, Ashton in Makerfield, Newton le Willows, and Leigh. We will always consider your request to stay on our practice list but please be aware that you may not always be able to stay with the practice.

Registration/ GP of Choice

As from January 2015 GP Practices were able to choose, under ‘GP Choice’, whether or not to offer patients from outside the usual boundary the opportunity to register as new patients under the proviso that the practice was not under any obligation to provide that patient with home visits or out of hour’s services.

The Practice took the decision in January 2015 not to offer this form of registration and therefore if you are currently registered with the practice and move to an address outside the practice boundary OR are a new patient wishing to register with the practice but reside outside the approved new patient intake boundary, you will not be eligible to remain registered or to become registered with the Practice.

GP Choice was to allow patients to visit GP practices closer to their place of work, school, etc. and as such was not deemed a necessity at The Family Medical Practice.

Boundaries are approved by NHS England.
