You do not need a Fit Note (Sick Note) in the first week of absence, instead you should ask your employer to provide a Self Certificate form for you to complete for this period.
If you have not seen a clinician for the condition you are unable to work with then you will need an appointment to assess the condition.
In most cases the clinician will be the GP – however you may be seen by an Advanced Nurse Practitioner or the Practice Nurse depending on the condition being assessed. The reception team will advise you on this matter.
Repeat Fit Notes can be requested via ASKMYGP – the clinician will issue a repeat Fit Note if appropriate to do so and will ask you to make an appointment to reassess your condition as and when necessary.
